The Community Health Worker will join the Health Visiting team within Public Health Nursing in Tiverton/Cullompton, focusing on improving the health and wellbeing of children aged 0-8 and their families. The role involves providing individual and group support in various settings, including homes and community venues, while collaborating with a multi-disciplinary team. Key responsibilities include conducting developmental reviews, aiding families with health-related topics like behavior management and nutrition, and delivering preventative and targeted interventions based on evidence-based practices.
