The Therapy Clinical Support Worker - Home First role at Buckinghamshire Healthcare NHS Trust offers a unique opportunity to join a developing team that assists patients transitioning from hospital back to independent living at home. The candidate will play a vital role in providing holistic, person-centered support, helping patients achieve their recovery goals through home visits and therapeutic treatments under the supervision of qualified therapists. Responsibilities include executing treatment protocols, managing mobility equipment, documenting progress, and collaborating with senior therapy staff to enhance patient care.
