Senior Mental Health Practitioner (RNMH) with Lincolnshire Partnership NHS Foundation Trust in Pinchbeck, Boston
An exciting opportunity has arisen for an Assessment Practitioner (Band 6) to work within the Spalding & South Lincolnshire Rural locality mental health service. The community mental health team provides comprehensive bio-psychosocial support to those of working age who are experiencing severe and enduring mental illness. It forms part of the Spalding & South Lincolnshire Locality mental health provision and is actively engaged in the community mental health transformation agenda. We are also actively working to develop the range of effective and evidence based social and psychologically informed interventions available within the locality and to embed the principles of the open dialogue approach within our practice. The successful candidates values and enthusiasm will be in keeping with this transformation. The post holder will be responsible for triaging referrals into the service, undertaking comprehensive psychosocial assessment and establishing collaborative goals. The post holder will provide professional advice and support to other agencies and members of the multi-disciplinary team To provide a high standard of collaborative, trauma informed, strengths based intervention, focussing on the goals and aspirations of the individual and their social network. To identify clear recovery focussed goals utilising Iroc, the HOPE toolkit and other appropriate measures to guide the teams interventions. To establish clear formulations of the individuals current needs. To empower individuals and their social networks, untilising their strengths in order to achieve their recovery. To work collaboratively with partner agencies across the wider health and social care locality provision in order to meet the needs of social networks. To provide high quality learning and development opportunities for both colleagues and students. To contribute to the maintenance of a psychologically safe environment in which the diverse views, experience and skills of all valued and utilised. About us To engage in comprehensive biopsychosocial assessment of the needs, strengths and goals of the individual and their social networks. To identify clear recovery focussed goals utilising Iroc, the HOPE toolkit and other appropriate measures to guide the teams interventions. To facilitate collaborative interventions with individuals and social networks in order to promote their empowerment, recovery and wellbeing. Comply with and carry out safe practice in accordance with Trust policies and procedures, appropriate Codes of Conduct, the Mental Health Act (1983) and other relevant national and local guidance commenting on policies, procedures and developments as necessary. To provide both formal and informal training, health promotion and advice to patients and carer/relatives regarding their mental health needs. To actively participate in the delivery of training opportunities for staff and students, including those from other teams and disciplines. To provide high quality practice placement education for nursing students acting as mentor as required. To contribute to the maintenance of a psychologically safe environment in which the voice, skills and experience of all are valued and nurtured. Whilst this post is focussed on assessment and formulation the post holder will be required, to engage in providing other aspects of direct care and intervention as required in order to ensure the provision of a safe and effective service.
