Band 8a: £59,490 to £66,239 a year inclusive of HCAS  PERMANENT 
Identify, implement and evaluate relevant Quality improvement projects.


Clinical

  • Work as an autonomous clinical practitioner and be professionally and legally accountable for all aspects of professional and clinical work, within agreed national, local and professional standards and guidelines
  • Use clinical reasoning and physical assessment to assess, diagnose and treat the needs of patients, presenting with a deterioration in health and wellbeing who may be at risk of a hospital admission.
  • Recognise cues, signs and symptoms to initiate investigations and interpret results to formulate a diagnosis
  • Formulate and implement a comprehensive evidence-based plan of care with the patient/relative/carers, promoting choice and empowerment of the patient
  • Provide expert clinical knowledge, skills, advice and health promotion to patients/ carers/relatives to support them in their own home
  • To provide/facilitate a range of appropriate care at home including acute respiratory exacerbations, urinary tract infections, pain management and bowel management as required
  • To act as an advocate for the patient to ensure health needs are met consistent with patient wishes
  • Prescribe medication appropriate to the patient needs and in accordance with evidence-based practice, national and local guidance and within professional scope of practice.
  • Evaluate treatments plans for patients with acute and/or complex needs and amend plans in response to patient treatment.
  • Use appropriate assessment tools such as NEWS2 to assess for deterioration,
  • prioritise health problems and escalate effective emergency care or urgent care as appropriate
  • Initiate appropriate and timely consultation and/or referral with appropriate medical professional when the problem exceeds own professional scope and practice
  • Work in partnership with community nursing services, allied health professionals, General Practitioners, adult social care, 111, ambulance service and relevant health and social care partners


Service Delivery

  • Be responsible for ensuring delivery of the rapid response service seven days a week through appropriate delegation and organisation of workload across the team
  • Use effective Communication methods and resources with clients, carers, team members, health care professionals and adult social care to promote collaborative working
  • Act as an expert clinical resource to members of the team by providing advanced clinical advice and support and intervention where necessary
  • Monitor, review and ensure delivery of the key performance indicators for rapid response and urgent community response service
  • Work collaboratively with the frontline service lead to develop and implement the Urgent community response standard across Your Healthcare community services.
  • Lead on service development and implement progressive ways of working in response to NHS and local plans
  • Practice within the framework provided by the YHC Policies, Procedures and Guidelines and those laid down by the professional regulatory body
  • Participate in the development and integration of care pathways, policies and procedures, which will influence service delivery and practice.
  • Record accurate legible health records
  • Establish robust and effective communication channels with clients, carers, team members and other health care professionals and agencies to promote collaborative working
  • Accurately record, review and report on service performance
  • Work in collaboration with southwest London integrated care system partners.


Personal and People development/Leadership

  • Provide leadership and support to team members and students
  • Facilitate and actively contribute to the development of team members through the annual appraisal process and use of personal development planning, one to one meetings, setting team goals.
  • Provide education and a variety of learning opportunities within the work area.
  • Participate in and contribute to the supervision of self and of the team according to the YH Supervision Policy in order to continuously improve quality of care.
  • Use evidence-based practice, research and development within practice in order to develop own and others knowledge and understanding.
  • Contribute to identifying and fulfilling personal and team objectives as part of the performance development review process according to the organisations policy.
  • Lead on the recruitment and induction of new team members
  • Monitor and review sickness absence of team members as per YH sickness absence policy
  • Facilitate a culture that develops the leadership potential of all staff.
  • Ensure team members regularly attend and contribute to monthly team meetings

  • To undertake audits related to clinical and advanced clinical practice ensuring that the outcomes of such audits are evaluated and that any changes required are integrated into practice.
  • To participate in audit, research and projects designed to promote safe autonomous practice
  • To contribute to the planning and development of policies, procedures and guidelines relating to clinical practice
  • To undertake the review and investigation of incidents as per the organisations internal process and policy
  • Evaluate service effectiveness and performance against local and national standards and implement changes where necessary to improve outcomes.

The Rapid Response Team is a supportive and friendly team of nurses, paramedics and therapists. This is an opportunity for an experienced and motivated autonomous practitioner to join and lead the community team, based at Hollyfield Road, Surbiton.

The team is part of the 2-hour urgent response service responding to adults who are experiencing a health crisis and require assessment and intervention to support them in their own home, rather than being admitted to hospital. The team assess patients in their own home, formulating working diagnoses and management plans. The team also work collaboratively with multidisciplinary professionals in order to manage complex health and social care needs.

The post holder will demonstrate safe, competent, advanced clinical assessment and decision-making skills and apply a broad range of clinical knowledge and clinical interventions. They will provide leadership to the rapid response team and work collaboratively with the urgent community response services to deliver the urgent community response standards
