Rochford District Primary Care Network (PCN) is seeking a full-time Data and Outreach Coordinator to support patient care planning across several medical practices, including Ashingdon Medical Centre and Wakering Medical Centre. The role requires strong administrative skills, SystmOne and primary care experience, and effective communication with various professionals. The ideal candidate should be self-motivated, organized, and able to manage workloads under tight deadlines.
