An exciting opportunity has arisen for a well motivated Senior Mental Health Practitioner to join the Knowsley CYPMHS team. We have one 12 month fixed term contract available which covers 0.8 wte (4 days per week).
You will have high standards of care and provide assessment, treatment and care co-ordination of children and young people referred into the service.
You will be expected to work as a member of the MDT and also be able to work well independently to provide person centred, evidence based care at all time.
1. Responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care.2. Act as Named Nurse as required3. Ensure that the nursing practices of the clinical area comply with the Trusts policies and procedures4. Participate and assist in setting standards of service user care based on sound research findings.5. Participate and assist in systematic monitoring and evaluation of nursing practice within clinical area6. Carry out nursing care based on the principles of individual care programmes/nursing process in accordance with specialist integrated Care Pathways.7. Assist in the development and implementation of integrated care pathways, including appropriate service user assessments and models of care.8. Support the implementation and maintenance of team working9. Participate in process of potential new admissions to the service, including the assessment of such individuals in accordance with the agreed care pathway and completion of quality reports where appropriate.10. Develop therapeutic relationships with service users and carers.11. Participate in case supervision to monitor standards of service user care and documentation.12. Participate in regular clinical supervision as per Trust policy.13. Participate in managerial supervision.14. Ensure the ward environment is conducive to promote service users health, safety and well being, thereby preserving and respecting each service users personal dignity, privacy, religious and cultural beliefs.15. Incorporate health promotion and health education into your practice.16.
