Older Adult Community Mental Health Senior Practitioner with States of Guernsey in Guernsey
The States of Guernsey are looking for a Senior Mental Health Nurse Practitioner to join the team at the Older Adult Community Mental Health Team which incorporates the memory Assessment service. The successful candidate will will hold a caseload of patients with a range of complex mental health difficulties, and provide assessment, treatment planning, risk assessment and management for patients and provide ongoing community support for families and carers, ensure that systems and processes are in place to provide integrated care pathways and seamless transitions between departments and services. They will also demonstrate knowledge skills and competence in undertaking memory assessments, and the provision of post diagnostic support and signposting. The post holder will be part of a community mental health team, based within specialist mental health services at the Oberlands Centre within the grounds of the Princess Elizabeth Hospital. Care and treatment will be delivered to older adults with a functional or organic mental health difficulty living in the community, either in their own homes or residential care settings. The Multi-disciplinary team is comprised of Community Psychiatric Nurses, Occupational Therapists, Support Workers, Consultant Psychiatrist and Associate Specialist. The post holder will hold a caseload of patients with a range of complex mental health difficulties, and provide assessment, treatment planning, risk assessment and management for patients and provide ongoing community support for families and carers. The post holder will ensure that systems and processes are in place to provide integrated care pathways and seamless transitions between departments and services. They will demonstrate knowledge skills and competence in undertaking memory assessments, and the provision of post diagnostic support and signposting. Undertake needs assessments and liaise with other agencies to develop care packages. They will demonstrate knowledge skills and competence in managing patients with a range of functional mental health conditions offering a range of interventions to optimise the persons functioning and management of symptoms. About us CLINICAL RESPONSIBILITIES To undertake assessment, development, implementation, and evaluation of complex programmes of care which improve the purpose and quality of a service users life. To autonomously initiate emergency interventions as patient/client condition dictates. To provide prescribed care without supervision, within the level of the post-holders experience, referring to more senior staff if necessary. To participate as team member, including supporting relatives and friends of service users together with providing support for service users. To maintain accurate records, both electronic and paper, of the care given in accordance with agreed standards. To act in accordance with their regulatory bodies professional code of conduct. Independently liaise and communicate effectively with members of the multidisciplinary team and other associated agencies. Demonstrate evidence-based practice. TRAINING AND EDUCATION Participate in contributing to the continuing development of the departments philosophy, training/education. To undertake in-house annual mandatory training in Manual Handling, Basic Life Support, Fire Safety and AED training. To undertake the role of health promoter and educator for service users and their families/significant others. To maintain and develop own skills to promote evidence-based practice, within the role of therapeutic engagement. To participate in clinical supervision. To assist in the learning, development and assessment of unqualified staff and junior staff. To undertake a formal mentorship role if an appropriate qualification is held. Comply with the conditions laid down by the regulatory body with regard to revalidation. LEADERSHIP/MANAGERIAL In liaison with the Clinical Lead, gain experience in providing leadership to junior staff. Deliver and receive regular clinical and management supervision. Will contribute to the provision of in- house training. Will attend all MDT meetings, clinical reviews and any Serious Untoward Incident (SUI) reviews and debriefs. COMMUNICATION To undertake the co-ordination of complex discharge arrangements through communication with multidisciplinary team members and associated agencies. To communicate effectively with service users and their significant others, taking into account their cultural background and cognitive functioning and abilities. Attend weekly ward reviews if necessary, MDT meetings and caseload supervision and reflective practice groups. To manage conflict between individuals, including staff, service users and their significant others. To act as an advocate for service users, significant others and more junior staff members. To ensure written communication complies with organisational and professional standards. To give due consideration to language and cultural differences of both patients/clients/relatives/significant others and colleagues. Demonstrate excellent communication skills in liaison with external agencies, partner organisations, and other teams. Always act with compassion and treat patients, families and carers with dignity and respect. Demonstrate adherence to the key values of the organisation which include Compassion, Accountability, Respect and Excellence (CARE). AUDIT/INFORMATION TECHNOLOGY To participate in clinical audit as required in department. With training, access organisational electronic information systems relevant to the role.
