The States of Guernsey is seeking a Senior Mental Health Nurse Practitioner for the Older Adult Community Mental Health Team, which includes the memory assessment service. The role involves managing a caseload of older adults facing complex mental health challenges, conducting assessments, developing treatment plans, and providing ongoing support to patients and their families. The practitioner will work within a multidisciplinary team at the Oberlands Centre, ensuring integrated care pathways and facilitating smooth transitions between services.

Key responsibilities encompass delivering evidence-based care, coordinating complex discharge arrangements, and participating in training and supervision of junior staff. The position also requires strong communication skills to advocate effectively for patients and manage conflicts, while adhering to the organisational values of Compassion, Accountability, Respect, and Excellence (CARE).
