Maidenhead Primary Care Network is seeking a Care Coordinator to join their multidisciplinary healthcare team, aimed at enhancing personalized and coordinated health and social care for a patient population of 70,000. The Care Coordinator will focus on identifying and supporting individuals, especially the elderly and those with long-term conditions, by navigating them through various health and care services. This includes collaborating with GPs and practice teams, ensuring patients receive appropriate support, and helping them manage their health effectively.
