As ward discharge Co-Ordinator the post holder will provide support in order to facilitate and drive the safe, effective and timely transfer of patients from the Trust to the most appropriate setting to meet the needs of the patient, reducing length of stay and maximising use of hospital beds.

To ensure best practice in the provision of a discharge service within the Trauma/Orthopaedic wards through the stipulation of a high quality, consistent, robust, proactive, and timely service.
To provide a single point of communication with patients/relatives/carers ensuring that the discharge processes are patient centred and consider the needs and wishes of the patient/relative/carer.
The post holder will be responsible for his/her own caseload working with support and supervision from the complex discharge coordinators when necessary.
