The Kent & Medway NHS & Social Care Partnership Trust is seeking a motivated and committed Senior Clinical Pharmacist for the South Kent Coastal community mental health team, located in Folkestone or Dover. This role is part of the innovative Kent and Medway Community Mental Health Transformation Programme aimed at supporting individuals with severe mental illness to lead independent lives within their communities. The ideal candidate should possess an independent prescribing qualification or be actively pursuing one, have a post-graduate pharmacy qualification, and experience in mental health. The pharmacist will be responsible for providing clinical pharmacy services, facilitating patient-centered reviews, and driving the optimisation of mental health medications in collaboration with primary and secondary care providers.

In addition to delivering high-quality pharmaceutical care, the Senior Clinical Pharmacist will oversee the management of medicines, support shared care arrangements, contribute to patient discharge planning, and engage in education and training for community mental health team staff. The role offers ongoing training, clinical supervision, and opportunities for professional development, including post-graduate qualifications and leadership courses. The successful candidate will work both independently and as part of a collaborative team, ensuring effective medication management and support for patients across different care settings.
