Competent Practice Participating in the assessment of health-related needs of patients and their families/careers. Contributing towards the formulation of a plan of care with the patient and their family/carers. Delivering direct care to patients in accordance with the Trusts policies and NMC Guidelines. Continually assessing and evaluating the patients condition and the effectiveness of the care delivered with a senior member of the team. Promoting a multi professional approach to maximise the patients health and independence utilising resources appropriately. Maintaining up to date knowledge of current nursing issues/practices. Working as a registered nurse, without direct supervision in the absence of the Ward Sister where necessary.
