The Upton Lane Medical Centre in London is seeking a Health Care Assistant (HCA) to join their multidisciplinary team within their GP practice. The ideal candidate must possess an HCA Care Certificate or equivalent, prior experience in a GP environment, and knowledge of EMIS systems. The role involves supporting health promotion and prevention initiatives, taking physiological measurements, and ensuring accurate patient record-keeping. The HCA will work closely with Nurse Practitioners, Practice Nurses, other HCAs, a Paramedic, and a Prescribing Pharmacist, while reporting significant patient issues to GPs or Lead Nurses.
Key responsibilities include confirming patient identification, preparing and maintaining clinical environments, providing appropriate patient information, and carrying out various administrative tasks. This position is crucial in delivering quality healthcare to the patient population while adhering to the established scope of practice. To work as a HCA, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The HCA will be responsible for a number of clinical areas such as, health promotion and health prevention.
You will be working in conjunction with skilled and experienced Nurse Practitioner, Practice Nurse, 2 HCAs, Paramedic, Prescribing Pharmacist& supported by our GP's.

Key responsibilities
- Check patient identification and confirm details with the patient.
- Take physiological measurements from patients as appropriate such as blood pressure, oxygen saturation, peak flow, pulse rate and rhythm, temperature, height and weight (body mass index)
- Prepare and maintain environments and equipment before, during and after patient interventions
- Give accurate and appropriate information to patients within own competence
- Ensure that anomalies are reported to a senior member of the clinical team
- Notify a GP/Lead Nurse of any significant patient issues
- Ensure full and accurate recording in the patient’s computerised records
- Carry out administrative tasks for example, but not limited to, call and recall activities and stock takes as directed by senior members of the service
