Are you a highly skilled nurse with a Master's degree, ready to take your career to the next level?
This is an exciting opportunity to join the team at North East Derbyshire PCN as an Enhanced Practice Nurse. The role is at the front line of the NHS/primary care, seeing acute presentations at GP surgeries. You will be responsible for assessing these patients, taking a history and examination and developing management plans whilst identifying any red flags that may need onward referral. Some cases may need follow up, and this will give you the opportunity to develop continuity and build relationships with patients and their families over time.
The successful candidate will be well supported in the role, all of our practices have experience of developing nurse practitioners "in house" and encouraging independent practice and there will be a GP available to discuss cases with if you need support. Whilst experience in prescribing would be preferred, we are also happy to support with the Non-Medical Prescriber's course if this is needed. We would also actively support any development of specialist interests that would benefit the PCN.
For a further discussion around the role please contact Sarah Buckley/Dr Tom Martin.
The postholder will act autonomously within their professional scope of practice providing care for patients face to face and via telephone consultations.
The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The practitioner will need to prioritise the needs of patients, making any necessary referrals for investigations in the appropriate manner.
The practitioner will be supported in their role by GPs within the practices within the PCN. There will be opportunities for upskilling and further training within the role if necessary. We anticipate that the successful candidate will be based at one or two practices within the PCN, subject to discussion around working patterns. Although we have a full time vacancy, we may consider part-time or job share.
About usPlease see Job Description attached for main duties of the Role.
The Successful candidate will be supporting the GP Practices across the PCN, helping to achieve the Network DES by supporting with the delivery of network Clinics.
