We are seeking to appoint a motivated Health Care Assistant to join the Primary Care Home Hub (PCH) based at Hamilton house in Lowestoft. The team delivers community healthcare to patients in their own home in the Lowestoft area.

This role focuses on the provision of nursing interventions. For example wound dressings, observations, bladder washouts. Delivering care, support and encouragement to patients in order to promote independence. Responding to patient and family needs. The Primary Care Home is a multidisciplinary team of Nurses, Therapists, Assistant Practitioners, Health Care Assistants, and Rehabilitation Support Workers delivering nursing, therapy and reablement/rehab services to adults in their own homes.

Do you have excellent communication skills, consider yourself to be caring and compassionate and want to make a difference to the patients experience on a daily basis, whilst being able to demonstrate how you embody our values and commitments and have a desire for personal and professional develop.

Ideally some experience in community work and NVQ level 3 in health care would be an advantage however consideration would be given to the right candidate.

• To work with patients towards self-care and independence.
• Take, record and report blood pressure, respiration, pulse rate, oxygen saturation levels, urinalysis. Be able to recognise and report the deteriorating condition of a patient.
• Undertake skin and nutrition assessments. Undertake wound dressing change.
• Administer insulin to stable patients (for those with NVQ level 3 in health care).
• Administer bladder wash out via urinary catheter.
• Participate in caring for patients at the end of life when appropriate.
• Ensure a high standard of record keeping is achieved in line with ECCH standards.

The service uses SystmOne as a clinical IT system to support mobile working.

The service operates 7 days a week 07:00 to 20:30 hours. Applicants will be required to work a variety of shifts including weekends and bank holidays. At times you may be required to work with another of the 4 PCH's from a different location to cover staff sickness, or capacity pressures. If not previously achieved you will undertake the nationally recognised Care Certificate programme on commencing employment.

• Undertake and record MUST and Waterlow assessments and take appropriate action.
• Be able to participate in wound care, skin tear pathway, venepuncture, collection of specimens or diabetic foot assessment as required.
• To report any concerns / changes in the patients condition to a registered nurse or appropriate Health care Professional without delay.
• Ensure a high standard of record keeping is achieved in line with ECCH standards.
• Engage in personal development, maintain competence, knowledge and skills commensurate with role.
• Take responsibility for the safe use of clinical equipment.
