NHS Gloucestershire Integrated Care Board is seeking a proactive and dynamic Case Worker (Team Lead) to oversee a small team that will facilitate communication between Continuing Healthcare (CHC) Clinical staff and care providers. This role involves non-clinical case management for individuals receiving NHS Continuing Healthcare and Funded Nursing Care, monitoring care provision, and ensuring the needs of these individuals are met. The successful candidate will work collaboratively with Nurse Assessors and Complex Case Managers, ensuring accurate record maintenance and supporting all aspects of care.

The ideal candidate should possess a background in various healthcare settings, strong organisational and leadership skills, and the ability to thrive in a fast-paced environment.
