The Continuing Healthcare Team is looking to appoint a dynamic, self-motivated Case Worker (Team Lead) to lead a small team of case workers who will provide effective and efficient liaison between the CHC Clinical staff and care providers. This is an exciting new role within the Continuing Healthcare Team which is a busy operational team within Gloucestershire ICB.
The Case Worker team will work alongside the team of Nurse Assessors and Complex Case Managers within a range of settings and will undertake the non-clinical case management of those in receipt of NHS Continuing Healthcare (CHC), Funded Nursing Care (FNC) and other sources of health funding. This will include liaising with placements, monitoring of care provision and changes to presentation. The Case Worker Team will support the Clinical staff to ensure the care provision meets the needs of an individual, maintain accurate records and support all aspects of work for those individuals in receipt of health funding.
The post holder will have experience of working within a range of different healthcare settings. You will have excellent organisational skills and be able to work independently and creatively, using a problem-solving approach. It is essential that you are motivated, with strong leadership skills, a team player and enjoy the challenge of working in a fast-paced environment. The successful candidate will need to be able to work well under pressure, managing competing demands and prioritise according to the requirements of the service.
The post holder will work alongside the CHC Nurse Assessors & Complex Case Managers within a range of settings undertaking the non-clinical case management of those in receipt of NHS Continuing Healthcare, Funded Nursing Care and other sources of health funding from Gloucestershire Integrated Care Board GICB. This will include liaising with placements, monitoring of care provision and changes to presentation which may affect an individuals eligibility. Work with the CHC Clinical staff and providers to ensure individuals are receiving appropriate care provision and regular reviews of their health needs.

The post holder will be expected to provide accurate and effective communications within and outside of the CHC service, dealing professionally and discreetly with enquiries from patients, relatives, carers, legal representatives, health & adult social care professionals and other organisations.
The post holder will undertake reporting and analysis of information to support delivery and development of the role.
The post holder will need to meet deadlines, collate data and maintain standards relating to the Key Performance Indicators and Quality.
