Chelsea and Westminster Hospital NHS Foundation Trust is seeking a Heart Failure Clinical Nurse Specialist to join their supportive inpatient heart failure team, which also spans West Middlesex Hospital. The ideal candidate will possess extensive experience in cardiology, particularly in heart failure care, and will have the capacity to work autonomously while managing their own caseload. Key responsibilities include overseeing nurse-led clinics, telephone consultations, and heart failure programs, providing education and support to both inpatients and outpatients, and engaging in the development of new services and digital initiatives such as virtual wards.

This role emphasises professional development, allowing the specialist to enhance their skills in drug titration, patient monitoring, and end-of-life care in line with established guidelines. The post holder will also contribute to service improvement through audits, quality activities, and strategic planning, while fostering collaboration across healthcare teams. Active involvement in research, dissemination of findings, and participation in local and national working parties will be key aspects of the role, aimed at advancing heart failure care within the Trust.
