East London NHS Foundation Trust is looking for motivated Band 5 Community Nurses to join the South Bedfordshire Primary Home Care Team in Dunstable. The role involves providing community nursing care, focusing on self-care facilitation for patients with long-term conditions, health promotion, and participation in innovative projects. Key responsibilities include contributing to Urgent Response and Virtual Wards initiatives.
Candidates should possess a valid driving license and access to a vehicle, as duties may require travel. Applicants with experience in adult community services are preferred, but newcomers who are eager to pursue a community nursing career are encouraged to apply. The position requires working a shift pattern covering 7 days a week from 08:00 to 21:00, and both full-time and part-time roles are available.
