East Cheshire NHS Trust is seeking an experienced Community Caseload Manager for the Knutsford District Nursing Team. This role involves working within a multi-disciplinary team to deliver high-quality, holistic health and social care to housebound patients across a semi-rural area. The successful candidate will have excellent communication skills and a minimum of two years post-registration experience, alongside the ability to manage patient caseloads effectively, ensuring comprehensive assessment and care planning.

The position requires a commitment to modernizing community nursing services in line with the NHS Long Term Plan, as well as collaboration with various healthcare professionals. Applicants should be prepared for travel as it is essential to the role, which entails 37.5 hours of work per week.
