The role of Clinical Pharmacist at North West PCN in Doncaster, under Primary Care Doncaster Ltd, involves working as part of a multi-disciplinary team to provide patient-facing pharmacy services across various medical practices within the Primary Care Network. The successful candidate will support chronic disease management, conduct clinical medication reviews, address prescription and medication queries, and enhance the safety and quality of prescribing. They will also collaborate with community and hospital pharmacies to improve patient outcomes and streamline workload, while providing education and support for self-management to patients.

Candidates must hold a degree in pharmacy, be registered with the General Pharmaceutical Council (GPhC), and preferably possess a Master's degree (MPharm) or equivalent.
