The Community Neuro team are seeking a motivated and enthusiastic Band 6 nurse to join our friendly, supportive team.

The Community Neuro team are seeking a motivated and enthusiastic Band 6 nurse to join our friendly, supportive team. You will be working to support the delivery of specialist nursing for people with multiple sclerosis, providing a comprehensive assessment and expert advice on the management of their condition. You will work closely with your multidisciplinary colleagues and have regular support and guidance from specialist nurses within the team.

The post involves working at a variety of bases within the local area.

  • The post holder will provide expert advice in the management of patients with Multiple Sclerosis (MS).
  • They will perform comprehensive assessment of patient needs, plan implement and evaluate care delivery according to changing healthcare needs ensuring that high quality patient centered care is delivered.
  • Work collaboratively with the multidisciplinary team and other agencies to ensure patient's needs are met.
  • Assist the specialist nurse in review and monitoring of treatment therapies.
  • Undertake nurse led clinics.

  • To triage calls that the nursing team receive to support prioritisation of caseload.
  • To provide a link in the care of people with long term neurological conditions between acute and community services.
  • Carry out holistic assessments of patients with long term neurological conditions and use sound clinical reasoning skills to determine treatment indicates and appropriate signposting.
  • To act as a resource for other healthcare professionals, and share good practice locally.
  • To support the delivery of self management programmes.
  • To be actively involved in service review and development.
