Livewell Southwest is seeking a dedicated Physiotherapist to join their Community Crisis Response Team in Plymouth, working up to 37.5 hours per week. This role focuses on providing urgent care to clients in their homes, aiming to prevent unnecessary hospital admissions during periods of acute illness. The successful candidate will be part of a supported multi-disciplinary team, responsible for managing a caseload of complex patients while engaging in holistic assessments and collaborative care strategies. The position offers opportunities for professional growth, including the supervision of junior staff and involvement in service improvement initiatives.
