The role of Staff Nurse at Sussex Community NHS Foundation Trust involves working in a 38-bed Intermediate Care Unit, catering to patients who require support in their recovery after an acute episode but are not yet ready to return home. The nursing team, consisting of Registered General Nurses (RGNs), Advanced Clinical Practitioners (ACPs), healthcare assistants, and rehabilitation specialists, focuses on maximizing patient potential and ensuring their safe transition back to their home environment. The position requires a collaborative spirit and offers a rewarding opportunity to contribute to patient pathways, enabling progress and providing support through setbacks, in a setting that has received an "Outstanding" rating in a recent CQC inspection.

As part of a 24-hour service, the Staff Nurse will participate in internal rotations and contribute to delivering high-quality, evidence-based care. The successful candidate will also engage in staff development and lead nursing teams, reinforcing a commitment to multi-professional collaboration in facilitating rehabilitation, self-management, and independence for patients throughout their care continuum. As an Intermediate Care Unit with 38 beds, we bridge the gap between home and acute hospital services. Patients who are too unwell or incapacitated in the short term to stay at home but not requiring acute intervention get admitted to our unit to help them recover and 'return to baseline' function.

In addition patients who have had an 'acute' episode but are not yet ready to return home will come to us for a short period of rehabilitation to ensure they are safe to return to their home environment and their abilities are optimised.

We are a team of RGNs, ACP ,Healthcare assistants, Physiotherapists, Occupational Therapists and Rehabilitation support workers who aim to maximise our patients' potential to enable them to return home. When that is not possible, we work closely with our adult social care colleagues to plan best outcomes.

We are looking for an RGN who enjoys working as part of a friendly and supportive team. It is hard work but very rewarding - planning the patient pathway, enabling them to progress and supporting them through setbacks is what we do best.

As this is a 24-hour service, internal rotation will be operating.
