As part of this team you will be responsible for forming effective relationships with the doctors, staff and patients.

An opportunity has arisen for a Practice Nurse to join our growing, Practice of 6,200 patients. We have a team of 3 GPs, 1 Practice Nurse and 1 HCA and 1 GPA.

We are a GP Training Practice working in collaboration with Keele Medical School.

We are a high achiever of QOF and use Emis Web Clinical system.

A compassionate yet steady and composed approach is vital with excellent communication skills and a passion for improving services for patients. Good decision making and clinical skills are vital. You should have competent IT Skills ideally with experience of EMIS Web.

We are looking for someone part time, approximately 20 to 30 hours and daily hours can be negotiable for the suitable applicant. Salary negotiable dependent upon experience in general practice.

Experience of working in general practice is essential.

Duties will include:

General Nursing duties

Childhood Immunisations

Immunisations and Vaccinations including COVID

Travel Health Clinics

Chronic Disease Management

Care Plans

Contraceptive Advice/Pill Checks

General Nursing Duties including; assisting with minor surgery/wound management

Cervical Cytology

ECG

Phlebotomy

QOF /QIF/Enhanced Services

Undertaking Audits

Oversee the HCA and training where applicable

As part of this team you will be responsible for forming effective relationships with the doctors, staff and patients. You will be required to undertake all general nursing duties in accordance with the practice protocols and establish programmes of care for patients groups with continual overall nursing requirements.

You will ensure that all nursing care delivered to the practice population is relevant, effective and of a high standard.

KEY RELATIONSHIPS

To work with members of the Primary Care Nursing Team, Multi-disciplinary Primary Health Care Team, and other agencies, statutory and voluntary, to improve the standards of health in the community and promote the health of the individual.

To promote a learning environment for staff, students, families, carers and patients as necessary.

KEY RESPONSIBILITIES

Support and enhance the patient care offered by the GPs

Advise, educate and develop ongoing healthcare with an emphasis on preventative measures.

Support & educate patients on their condition and development of self-management plans where appropriate.

Refer to other PCHT members and other professional agencies as appropriate

Liaise with other members of the community nursing team and work as part of an integrated team leading and managing any necessary adjustments and changes in practice.

Implement and evaluate the agreed programme of patient care, whilst acting on and reporting any changes as necessary to the relevant team member

Participate in the development of nursing policies and protocols with reference to best practice.

Management of change in both patient care and ensuring best practice by nursing team.

Attend training as required and to complete an annual Personal Development Plan

Identify individual training needs

Support personal development of members of Practice Nursing Team

Maintain standards of care for patients with acute and chronic medical conditions using national and local guidelines where appropriate

Work autonomously in providing the care and management of chronic disease care provision in line with local/national guidelines.

To maintain and regularly update clinical record.

To regularly evaluate patient care.

Knowledge and understanding of clinical computer systems and the generating of clinical audit and providing information.

To be involved in ongoing clinical supervision

To accept responsibility and report any necessary maintenance on all nursing equipment used.

To participate with the induction, training and continuous clinical support of the nursing staff in the practice, Staff Nurses and Health Care Support Workers as appropriate.

Stock control of vaccine and other nursing requirements as necessary

To maintain confidentiality at all times

Attend staff meetings and represent the best needs of the patient (patient advocate) and the nursing team.

Ensure there is adequate cover and provision of nursing care during periods of staff absence/annual leave/sickness.

Responsible for the management and organisation of the recall system for

Childhood Immunisation

Clinical Screening

Chronic disease management

Liaising with the members of the community nurse team in the management of ensuring the uptake of

Childhood Immunisation

Cervical Screening

Flu Vaccination

Support GP and Patient in medicine management and medicine review.

General Nursing duties to include:

Smears and vaginal swabs

To perform ECGs

Change dressings, remove sutures

Syringe ears if deemed necessary

Administer injections, immunisations and vaccinations in all areas

Travel advice and vaccination

To prepare and assist with minor operations

To initiate and perform routine diagnostic tests, interpret and refer as necessary

Family planning

Stock control

Running specialist clinics e.g. Asthma, Diabetes, CHD etc.

Promotion of health education and provision of life style advice

Education on immunisation

Staying on top of QOF and QIF

GENERAL RESPONSIBILITIES

  1. To actively participate in annual appraisal, produce Personal Development Plan to identify individual training and development needs, and attend all mandatory training.
  2. To take personal responsibility for any Health and Safety issues and obligations under the Health & Safety Work Act. Also be aware of and comply with other relevant legislation and policies e.g. Fire Regulations
  1. Uphold all aspects of Practice policies and procedures, and legal requirements in relation to personal conduct.
