The role of PCN Care Coordinator at The Confederation, Hillingdon CIC in Uxbridge involves proactively managing a caseload of patients, including the elderly and those with long-term conditions, to improve their care coordination and support within the health care system. The Coordinator will work closely with GPs and practice teams to develop and review personalized care plans, facilitate access to services, and ensure effective communication between various health professionals, ensuring patients receive timely and appropriate care.
