This is an exciting opportunity to be part of an expert and supportive therapy team at Ascot House Intermediate Care Unit, which is an integrated, multi-professional and multi-agency Intermediate Care service based in Sale, Trafford.
You will be an integral member of the multidisciplinary team who work together to deliver an effective and safe Intermediate Care service.
We will rely on you to provide timely specialist physiotherapy assessments and treatments to patients who are admitted to the unit.
You will follow agreed protocols for ensuring appropriate and timely input and ensure pathways of care and communications with all agencies working with the service to deliver effective patient care.

To undertake physiotherapy assessments and formulate appropriate, patient specific treatment plans
Completion of detailed falls assessment and management plan in line with NICE guidance
Delivering reablement and rehabilitation interventions to patients to help them recover from their hospital admission and safely return to their home address
Working in partnership with OT's and support workers to deliver functional rehab in order to promote independence and facilitate timely discharge planning
Completion of clinical triage of referrals to determine if patients are suitable for admission
Completing moving and handling assessments of patient on admission, post fall or due to a change in condition
Prescribing and completing exercise programmes with patients
Delegating interventions to support and junior staff
Completing home visits when required and discharge home visits for all patients
Providing supervision and peer to peer support to junior, support staff and students
