The Health & Wellbeing Coordinator role at Gloucestershire Health and Care NHS Foundation Trust involves supporting individuals facing frailty, social isolation, and long-term health conditions through a case management approach aimed at enhancing their quality of life and independence. The Coordinator will collaborate with Community Matrons, GP Practices, Integrated Community Teams, and external providers to design personalized care plans for patients, guiding them to access appropriate health, social care, and voluntary sector services.

In addition to acting as a single point of contact for the patients, the Coordinator will build strong relationships to help identify health declines early and support individuals in navigating community services. The role requires effective communication and collaboration with a Multi Disciplinary Team, contributing to the overall care delivered by professionally registered staff while facilitating a proactive advisory approach for the targeted patient cohort in Gloucester, Cheltenham, and the Forest of Dean.
