The Torbay and South Devon NHS Foundation Trust is seeking a Healthcare Support Worker to join their community Paignton Health and Well-Being Team. In this role, the successful candidate will work across services such as Community Nursing, Intermediate Care, and Community Matrons, providing high-quality care to housebound patients. The position involves being part of a multi-disciplinary team, effectively communicating with patients and their families, documenting care activities, and collaborating with other healthcare professionals.

Key responsibilities include supporting patients in gaining confidence and independence, educating them on equipment usage, and participating in team meetings to enhance communication within the team. The Healthcare Support Worker will also adhere to clinical governance frameworks, respond to incidents, and promote a safe environment.
