The Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a dynamic Band 6 Frailty Practitioner for their Frailty Support Team, primarily based in Totton but requiring flexibility to work across Eastleigh, Fordingbridge, and Lymington. The role is suited for registered nurses, therapists, or paramedics who can drive for work and are committed to providing exceptional care for acutely unwell adults in their homes. The team operates seven days a week and collaborates with various healthcare professionals to offer rapid response services aimed at preventing hospital admissions and delivering personalized, evidence-based treatment plans for patients experiencing frailty.

In this role, the Frailty Practitioner will conduct advanced assessments, develop care plans, and manage complex cases, ensuring a person-centered approach that incorporates the involvement of patients and their families. The position emphasises service improvement and collaboration within a multi-disciplinary team, making it an ideal opportunity for professionals passionate about enhancing healthcare delivery in the community.
