The Powys Teaching Health Board is seeking a highly motivated specialist practitioner for the role of CAMHS Crisis Hwb Practitioner & Outreach Worker, a fixed-term position for 3 months to support a sanctuary Hwb pilot. The successful candidate will be part of the CAMHS crisis, assertive outreach, and home treatment team, providing community-based support to children and young people up to the age of 18 experiencing mental health crises. This role involves offering intensive home treatment in collaboration with service users to prevent hospital admissions, as well as facilitating early discharges from inpatient units.

The position is ideally suited for candidates who can communicate in Welsh, though English speakers are also encouraged to apply. The practitioner will primarily work from the Crisis Hwb, creating a safe and supportive environment for children and their families, and will be part of a team providing care seven days a week, year-round.
