The Midlands Partnership NHS Foundation Trust is seeking two Care Act Assessors, Band 4, to join the Cannock Adult Social Care Community Teams. This role is ideal for individuals considering a career in adult social care, as it provides opportunities for skill development in assessment and care management. The successful candidates will work collaboratively with service-users, carers, agencies, and professionals to conduct annual reviews, assess needs, plan care, and provide information in accordance with The Care Act (2014) and Mental Capacity Act (2005). A full induction and training will be offered, although familiarity with local support services would be beneficial.

Candidates must be motivated, possess strong communication and organisational skills, and have proficient computer abilities. A clean driving license and access to a vehicle for work-related travel are required, as the role involves travel across Staffordshire. The position offers a flexible work schedule with the option of a 9-day working fortnight, and successful applicants will participate in the Social Work Learning Academy for continuous professional development.
