CASELOAD MENTAL HEALTH MULTI-DISCIPLINARY
The Schools Mental Health Practitioner role at Hammersmith & Fulham, Ealing & Hounslow Mind offers a unique opportunity to work in a multi-disciplinary team focused on improving the emotional well-being of children and young people (CYP) aged 5-25 within educational settings. The successful candidate will deliver evidence-based low-intensity interventions, including one-on-one and group sessions, for those experiencing mild to moderate mental health difficulties, and will promote a whole school approach to mental health through psychoeducation and training for students, parents, and educators. Key responsibilities include assessing needs, developing intervention plans collaboratively, conducting risk assessments, maintaining clinical records, and engaging in regular supervision and professional development.

Candidates are encouraged from various mental health backgrounds, including Education Mental Health Practitioners and CAMHS nurses, and should be committed to inclusivity and equal opportunities while working with diverse populations. This role requires strong collaboration with local authorities, educational institutions, and other mental health services to enhance access to support, facilitate effective communication, and ensure the well-being of all students.
