The Worcestershire Acute Hospitals NHS Trust is seeking a part-time Colorectal PSFU Coordinator to join their General Surgery team at Worcester Royal Hospital, with some flexibility for home working. This role is essential in providing multidisciplinary support for patients diagnosed with colorectal cancer, focusing on their physical, social, and psychological needs throughout their treatment journey and aftercare support. The successful candidate will work closely with Clinical Nurse Specialists (CNS) to coordinate the Patient Centred Follow Up (PCFU) pathway, assist with patient inquiries, provide administrative support, and facilitate health and well-being programs.

Key responsibilities include managing patient appointments, delivering health education, maintaining communication with patients and healthcare providers, and participating in service development initiatives. The Coordinator will also monitor compliance with remote surveillance pathways and contribute to clinical audits and research initiatives. Strong organisational, communication, and interpersonal skills are required, alongside a commitment to patient advocacy and continuous professional development within the cancer services domain.
