The Assistant Practitioner role at Care Plus Group's Community Inpatient Unit in Grimsby involves supporting a multi-disciplinary team in a bed-based rehabilitation facility. This unit aids individuals in their recovery after illness, surgery, or trauma by promoting independent living and facilitating hospital discharges. The Assistant Practitioner will be responsible for coordinating shifts, liaising with healthcare professionals, and implementing personalized support plans. Additional duties include assisting with admissions and discharges, supervising staff, and managing day-to-day service operations.

Candidates must possess a Foundation Degree or Higher Education Certificate in Integrated Care or a related field, or a Level 3 in health or social care with a commitment to progressing to Level 4. The position requires at least two years of experience in health or social care, proficiency in Microsoft Office applications, and the ability to work on a rota, including evenings and weekends.
