The Norfolk & Suffolk Foundation NHS Trust is seeking a qualified Band 4 Assistant Practitioner or Nurse Associate to join their Crisis Resolution Home Treatment Team (CRHTT) in Ipswich. Ideal candidates should be highly motivated individuals passionate about patient care and committed to involving Service Users and Carers in their approach. The role involves assessing, managing risks, planning care, and coordinating support for Service Users throughout their recovery journey. Working in a collaborative, supportive team, the Assistant Practitioner will also engage in various projects aimed at improving health care quality.

Candidates must hold relevant qualifications, including a Care Certificate and a Level 5 academic qualification in Health & Social Care or Mental Health, along with a full driving licence for travel purposes. The position requires flexibility in working hours, including some unsociable shifts, weekends, and holidays. Working alongside our Clinical Support Workers and Registered Nurses, you will work a combination of shifts including unsociable hours, some weekends and bank holidays.

You will be responsible for assessments, risk management, care planning, and co-ordination of care delivery. You will support the Registered Nurses with the day to day activities and provide support to service users throughout their recovery journey.

Working within a well-resourced team, supported with regular supervision and opportunities to work in partnership with health and social care in the wider multi-disciplinary service, we want to develop the role of an Assistant Practitioner or Nursing Associate to enable participation in a wider range of opportunities including change management projects and quality improvement.

Care Certificate

Foundation Degree in Health & Social Care / Mental Health at L5 or Apprenticeship in Health & Social Care / Mental Health l5 or Alternative (relevant) Academic Qualification at L5 (degree)
