An exciting opportunity has become available in the East Integrated Neighbourhood Team (INT) for a senior administrator to join our Camden Integrated Community Healthcare (CICH) workforce in full time capacity. As a Multidisciplinary Meeting (MDM) Coordinator working in a Camden INT, you will play a crucial tole in connecting all members of the Borough-wide multidisciplinary teams, including GP practices and other health and social care partners. We are proactive in looking for innovative people who will facilitate the delivery of excellent integrated working, as well as support the administrative functions for the smooth running of the East Camden Integrated Community Healthcare Team, working closely with the Operational Lead and other administrative and clinical staff.

Following the NHS long term plan, CICH is proud to lead on putting neighbourhoods at the heart of integrated care. The service is set up to deliver health interventions within the framework of the neighbourhood model, working in partnership with other key primary care providers and the local voluntary care sector organisations. This role will be based at East Integrated Neighbourhood team based at Kentish Town Health Centre, however the post holder would be expected to work across our Camden teams according to the needs of the service.

Co-ordinate and facilitate the function of all MDM meetings across the Neighbourhood and cover for colleagues across Camden; which will include the Borough MDM, 3 neighbourhood meetings in the respective neighbourhood of Camden (North, South and West), and be supportive as an integral member of co-ordinated complex care for frail and elderly patients in Camden.

Provide administration support to the Older Person's MDM members as part of the patient pathways across Camden and liaise with cross border MDM's i.e. Islington, Haringey, Enfield, Barnet, to ensure all referrals are managed accordingly and in line with any Memorandum of Understanding for Out of Area/ Cross Border Patients may be in place at that time.

Provide administrative support to the East CICH team in collaboration with other administrative staff. Training and support will be provided, as well as regular supervision.

Keep an up to date list of all MDM members names and emails, removing those from the list who leave the group.

Manage all referrals for review by the MDM. The referrals are received from Health and Social Care professionals.

Inviting the referrer and patients GP to relevant MDM meetings where the patient will be discussed.

Ensure that actions are followed-up using action logs.

Ensure that the patient case list is populated and kept up to date.

Email respective care plans and referrals to all MDM members, 24-48 hours ahead of the meeting.

Ensure all referrals are present on EMIS web and that EMIS is kept up-to date.

Providing relevant caseload data and reports.

Cover for colleagues across Camden when needed to co-ordinate and facilitate the function of MDM meetings across the Neighbourhood.
