Saddleworth Medical Practice in Oldham is seeking a Practice Nurse to join their experienced team on a part-time basis, working 30 hours per week. The ideal candidate will be a flexible and approachable team player who can manage patients with long-term conditions effectively through evidence-based practices. Key responsibilities include overseeing infection prevention and control, chronic disease management, and health promotion. The Practice Nurse will also develop care management programs for elderly and housebound patients, assess healthcare needs, implement vaccination programs, and review patient medications while ensuring adherence to clinical policies and procedures.

In addition to primary clinical duties, the Practice Nurse will be involved in audits, staff training, and the development of nursing protocols. Support for the clinical team in emergency situations, recognition of mental health needs, and liaising with external services will also be essential. The successful candidate will contribute to continuous improvement in patient care and ensure compliance with practice policies and standards.
