Wrightington Wigan & Leigh Teaching Hospitals NHS Foundation Trust is looking for a dedicated Community Cardiac Rehabilitation Nurse to join their Ashton, Leigh, and Wigan Community Cardiology Service. This permanent, part-time role involves working within a multidisciplinary team to provide evidence-based care for patients diagnosed with myocardial infarction or coronary revascularisation. The successful candidate will have significant cardiology experience, ideally at Band 6 or above, and a strong commitment to supporting patients through education and rehabilitation to improve their quality of life while preventing unnecessary hospital admissions. Key responsibilities include managing a caseload, providing specialist nursing advice, undertaking clinical assessments, and contributing to the development of clinical practices and policies.

The role requires excellent communication skills and the ability to work autonomously within a community setting, in partnership with patients, families, and healthcare colleagues. In addition to clinical duties, the nurse will lead audit activities, participate in multidisciplinary meetings, and support the continuous improvement of service delivery. Candidates should be prepared to engage in non-medical prescribing training within their first year.
