The Exeter City Primary Care Network (ECPCN) is seeking a Health Coach - Patient Navigator to support their same-day service for medical to low-risk patients, emphasizing the proper utilisation of healthcare resources. The role involves working within a multi-disciplinary team to educate and coach patients through various communication methods, such as text and phone, directing them towards appropriate community services and healthcare pathways. Key responsibilities include reviewing emergency department attendances, analyzing patient behaviors, crafting patient educational messages, and coordinating interventions to promote better healthcare navigation.

This temporary position, part of a six-month pilot in collaboration with Devon Integrated Care Board (ICB), aims to enhance patient care by minimizing reliance on inappropriate services and facilitating access to local health resources. The successful candidate will contribute to the team's service development, engage in data collection for performance evaluation, and participate in meetings for continuous improvement of patient care and service efficacy, ensuring a holistic approach that integrates physical, mental, and social health needs.
