The role of "Exercise Physiologist" at Wirral Community Health and Care NHS Foundation Trust in Birkenhead involves supporting a multi-disciplinary team to manage and develop comprehensive cardiac rehabilitation and prevention programs across multiple sites. Key responsibilities include managing a patient caseload from referral to discharge, assessing functional capacity, and prescribing exercise for patients with diverse complexities. The position also includes providing exercise guidance and assessment as part of the Tier 3 weight management service for individuals on the bariatric pathway. Within a multi-disciplinary team assist in the co-ordination and delivery and development of a comprehensive cardiac rehabilitation and secondary/primary prevention programme as part of Wirral Community Cardiology Services across three main sites in Wirral.
The role will include managing a caseload of patients from referral though to discharge as well as the assessment of functional capacity and the prescription of exercise for a wide range of complexities. The role also involves working within the Tier 3 weight management service, delivering exercise guidance and assessment for those accessing the bariatric pathway.
