There is an exciting opportunity to join the Haringey GP Federation to support four of Haringeys Primary Care Networks (PCN) as the Primary Care Network and Service Manager.
The role is hybrid and will enable PCNs to deliver change, build relationships with local stakeholders, and meet all the requirements of the PCN DES, including the quarterly finance and reporting. The role will also support various Federated projects supporting preventative health care management in primary care.
Additionally, the role will also plan, develop and deliver services that aims to support Haringey Primary Care Networks and Member Practices. Experience of delivering services is vital to ensure our projects support both the population and primary care within Haringey. Some projects will be commissioned over a long period and some for a minimum of 12 months or less. This role will be responsible for managing a range of services, or at least one large-scale service.

N15 PCN, which is made up of 5 practices in the South-East of Haringey
East Central PCN, which is made up of 4 practices.
North Central PCN, which is made up of 6 practices.
South-West PCN, which is made of 4 practices.
The service management element of this role will:
Plan and manage Services and ensure that the portfolio of tasks/projects is delivered effectively.
Adopt a project management approach to ensure that all projects are reported to appropriate parts of the organisation and to external bodies.
Take a lead in the coordination of training & development and recruitment activity for the Projects.
Oversight and management of all aspects of the Project budgets.
The postholder will work directly with each of the PCNs Clinical Director and member practices to support and facilitate the co-production and mobilisation of services in line with the PCN Network Contract Directed Enhanced Service specifications, and any other initiatives the PCNs are pursuing.
The postholder will facilitate the development of cross sector partnership working between the PCNs and other key stakeholders including commissioners, community services, acute trusts, local councils, mental health, community groups and the 3rd sector as well as other organisations as relevant e.g., housing, education, voluntary, etc. to support delivery of the Network Contract Directed Enhanced Service (DES).
