Job Purpose
Networks of general practices, currently constituted as Primary Care Networks (PCNs) under the national Network Directed Enhanced Service commissioned by NHS England, are a key building block of the health and social care system. The PCNs mission is to strengthen general practice and work with communities and health and care system partners to personalise care, improve population health and reduce health inequalities.
This role, employed by Primary Care Doncaster (PCD) and embedded in the PCN, is responsible for the development and implementation of the PCN Strategic Plan which sets out the ways in which the PCN will carry out its mission and in particular supporting the adoption of new technology and other initiatives to improve the care offer and enabling PCN staff to work more effectively to support the sustainability of general practice services and support successfully making changes and improvements to ways of working at a PCN and at-scale level. The postholder will manage and co-ordinate the activities of the PCN so that its strategic objectives are met and will provide leadership to the network alongside, and under the supervision of, the Clinical Director.
To improve adoption of new technology to enhance patient access and experience and increase PCN productivity
To build relationships and facilitate collaboration between practices to support the delivery of care to each other's patients (for example to delivery of enhanced access or other hub type working arrangements)
To review and improve the PCN's digital maturity
To use data, and improve data quality to:
o understand demand, capacity and activity and drive improvements in patient experience of access, operational efficiency, and staff experience at work
o support population health management
o improve care quality and PCN operational efficiency
o understand the type and intensity of support needs of their PCN and coordinate this support, including through OD programmes
o support the effective adoption of national and local initiatives, including integrated working at neighbourhood and place level to improve access to services for patients
o ensure that digital and operational PCN transformation is embedded in, and aligned with, ICS and national strategies
About usTo work closely with the practice managers of the core member practices to improve practice resilience and access to excellent care using digital resources and quality improvement techniques
To develop good working relationships with the ICB and other IT and Digital teams to identify opportunities for support and development.
To engage with communities and system partners in the North Locality and beyond in order to maximise their input into the plans and services of the PCN
To communicate the work and plans of the PCN to practices, communities, and system partners to facilitate engagement and ensure that the PCN has a very strong reputation for openness, innovation, and success
To be responsible for the day-to-day communication and engagement with patients and the public via use of social media
Develop the PCN social media and websites in conjunction with the PCD Communication and Engagement Officer
Act as first point of contact for complaints regarding PCN directly delivered services
To supervise the management of clinical services provided at PCN level and identify opportunities for quality improvement and new initiatives initiating bids for resources and service delivery
To be responsible for short, medium, and long-term transformation planning
Provider oversight of the PCN Professional Support Team workload and day-to-day operational duties and responsibilities including the recruitment and selection process, acting as first point of contact for individual concerns and performance management arrangements where Team members are not directly line managed by the postholder
To identify opportunities for digital optimisation and understand the impact within the PCN
To be aware of the national digital agenda and how this impacts the PCN in terms of prioritisation, meeting national and contractual targets, benefits to the local population and clinical services
To develop the PCN strategic plan in the light of changing circumstances and ensure its alignment with the Doncaster Place Plan and other local and Integrated Care System priorities
To manage the implementation of the PCN strategic plan in collaboration with the core member practices, Primary Care Doncaster and system partners in North Locality and beyond
To collate, analyse and use population and service data in order to inform PCN decision- making and monitor progress in conjunction with business intelligence support from Primary Care Doncaster and other system partners
To manage, develop and expand the PCN Professional Support Team in conjunction with workforce, contracting and HR support from Primary Care Doncaster or the relevant host practice to maximise its effectiveness in achieving the PCNs strategic objectives
