We welcome applicants seeking employment for 28 - 32 hours per week.

As a Nurse in the frailty team, you will provide high-quality nursing care to frail and elderly patients, focusing on managing long-term conditions, wound care, and supporting proactive hospital discharge reviews. You will work within a multidisciplinary team, liaising with GPs, nurses, paramedics, pharmacy technicians, and voluntary services to ensure integrated care for complex patients.

Deliver high-quality nursing care to frail and elderly patients, focusing on long-term condition reviews, wound care, and frailty management.

Work with colleagues in the frailty team to identify patients who would benefit from proactive review, What matters to me model.

Provide wound care support, especially when district nursing services are not accessible.

Conduct proactive reviews for patients discharged from the hospital, ensuring safe and effective follow-up care.

Assist with annual vaccine campaigns for frail patients, flu & covid vaccinations.

Build strong working relationships with GPs, paramedics, and other members of the frailty team to ensure integrated care.

Work alongside a multidisciplinary team to ensure comprehensive and cohesive patient care. Liaise with community partners, such as district nursing, palliative care teams, the voluntary sector, and the Integrated Care Therapy team, to improve patient and carer outcomes.
