This is an exciting opportunity to join our integrated Access Team within Navigo. If you are an experienced crisis clinician with strong leadership skills and are looking for your next challenge - this role plays a pivotal part in improving the lives of the people using our services, supporting them to access the most appropriate service(s) for their needs. The role of the Access Team Shift Manager is to facilitate the safe operation and management of the Adult Access team during allocated periods of responsibility. The Access Team comprises integrated Crisis & Hospital Liaison (ICHL), Single Point of Access (SPA), Home Treatment (HTT) and Well Together services. The role involves a high level of clinical and management responsibility, requiring sound clinical judgement, the ability to listen & communicate effectively and the ability to guide and inspire a team of professionals to best meet the needs of the people using services, particularly when there are competing priorities and complex circumstances.

Supported by the wider management team, the post holder will role-model evidence-based, person-centred and trauma-informed assessment and intervention in accordance with the services operational policy, national guidance and legislation. Working with and accepting referrals from other services both within and outside of Navigo.

Clinical practice and patient care

Ensure practice of self and team reflects the assessment response times and other metrics and outcome measures;

Design, deliver and evaluate care plans;

Attend and sometimes lead appropriate appointments and meetings, including MDTs, follow-up and assessment appointments, team meetings, etc.;

To be responsible for undertaking full mental health triage and assessment of care needs of patients presenting with complex clinical conditions;

To determine management plans, and where appropriate, deliver short term interventions for people who are referred with complex mental health needs based upon a comprehensive biopsychosocial, risk and needs assessment of that individual;

To determine and implement the appropriate treatment and risk management pathway for assessed individuals

Communication

Provide and facilitate comprehensive handover, and ensure appropriate and timely information sharing;

Where necessary, ensure timely, good quality information, shared in the most appropriate manner, to management, other teams & services, safeguarding, colleagues, people using services and other people involved in the care of those using our services

Analytical

Use and evaluate data, evidence and other information to guide positive outcomes for patients and to ensure the delivery of an efficient and safe service

Planning

Manage own time and priorities effectively, and support the team to do the same

Policy and legislation

Contribute to policy writing and review;

Ensure compliance with policy, guidance and legislation, including but not limited to:

iii)Information Governance and GDPR (Data Protection Act 2, Caldicott, etc.)

People, service and site management

Undertake basic people management duties, in liaison with and under the direction of the Business, People & Performance Manager

Maintain and support others to maintain mandatory training, and continuous professional development;

Take ultimate responsibility for the shift management of the Access Team and act as site lead for Harrison House when on duty;

To cover the 24/7 Access Team rota, and on-call rota;

To provide on-call support to services across the organisation, including advising and supporting teams to resolve issues that arise;

To supervise and manage the daily workload in such a way to maximise efficiency, ensuring that duties are discharged safely and effectively. This includes the processing, evaluation and allocation of referrals as appropriate to the teams function, and an efficient caseload management system;

To role model and embed practice in accordance with our values, including that of attitude, conduct, language and clinical practice;

Participate in and undertake managerial, live and clinical supervision;

Deputise for short periods of time in the absence of senior management

Information

To undertake investigations in-line with the relevant organisational policy or national guidance, including complaints, Incidents, serious incidents and structured judgement reviews;

Ensure accurate, comprehensive and relevant records;

Undertake internal clinical audit

Research

Conduct research and evaluation in order to support informed continuous development and improvement to the service and the team members
