The Norfolk Community Health and Care NHS Trust is seeking a Community Assistant Practitioner for its Diabetes Team, offering a part-time position of 18.75 hours per week in either Norwich or North Norfolk. The successful candidate will work under the supervision of a Diabetes Specialist Nurse, managing their own caseload and delivering care for individuals with diabetes in various locations, including clinics and patients' homes. Responsibilities will include conducting diabetes reviews, performing venepuncture and foot checks, providing education to patients and their families, and ensuring high standards of care.

Candidates need to demonstrate flexibility, strong communication skills, and a commitment to patient safety and support. A full UK driving license and access to a vehicle are required for independent travel around the area. The role emphasises teamwork within a supportive environment and encourages ongoing professional development through training and participation in quality assurance initiatives.
