As a Case Manager in the SWPC care, the post holder will work as part of a dynamic team in delivering an effective service to ensure safe and effective care pathways are in place for service users of specialised health care settings within the NHS and Independent sectors. The post holder will be required to support clinical staff and managers across Devon, Plymouth and Cornwall and as appropriate across the South West region and out of area.
Working as part of the mental health team responsible for the provision of care for specialised mental health, the post holder will provide clinical support for a designated caseload to enable the ongoing therapeutic interventions of service users in specialised health care. The aim of this clinical support is to have oversight of robust access assessments, ensure that each service user is placed in the least restrictive setting with the appropriate level of supportive interventions to enable their eventual discharge into the communitywhere this is a safe option or maintain their individual needs if they remain in a care setting.
The role will support the business in driving transformation as well as value for money in planning, commissioning and service delivery.
Key Working Relationships
Operate effectively in a flexible and demanding environment and proactively engage with key internal and external stakeholders.
Communicate and provide highly complex information to a wide range of internal and external stakeholders. (verbal, written and numerical)
Deal with potentially aggressive/antagonistic situations as required.
Work and engage constructively with stakeholders on a range of business sensitive issues.
Nurture key relationships and maintain networks internally and externally.
Apply a structured change management approach and methodology in relation to change management.
Partnership and cross boundary working
Work across multiple providers within the NHS and Independent Sector to support delivery of successful outcomes for service users
Work with all other Regional case managers in the management of particularly complex cases such as personality disorder and those with learning disabilities as well as delayed discharges.
Work in collaboration with services within the South West Region to manage relationships with CCG commissioned providers that provide the referral and aftercare components of the pathway
Work in collaboration with local authorities, third sector and other statutory bodies (such as MAPPA, Police, HMPPS, MoJ and CJS) in the management of effective usage of the specialised mental health services.
