The Practice Nurse team at Stanhope Mews West Surgery are responsible for delivering high-quality nursing care to our 16000 patients. The role involves a mix of direct patient care, health promotion, chronic disease management, and administrative duties.

The Nurses work as part of the wider practice team to ensure comprehensive care and support for patients.

  1. Direct Patient Care:

    • Conduct patient consultations, including taking medical histories and performing physical examinations.
    • Administer vaccinations, including childhood immunisations, travel vaccines, and flu shots.
    • Perform diagnostic tests, such as blood pressure monitoring, blood tests, and ECGs.
    • Provide wound care, dressings, and removal of sutures.

  2. Chronic Disease Management:

    • Manage and monitor patients with chronic conditions such as diabetes, asthma, and hypertension.
    • Develop and review individual care plans in collaboration with patients and other healthcare professionals.
    • Educate patients on managing their conditions, including medication management and lifestyle changes.

  3. Health Promotion and Disease Prevention:

    • Conduct health assessments and screenings, including cervical smears, sexual health services, and smoking cessation support.
    • Provide health education and promotion activities to encourage healthy lifestyles.
    • Offer family planning advice and contraceptive services.

  4. Administrative Duties:

    • Maintain accurate and up-to-date patient records using the practice's electronic health records (EHR) system.
    • Assist in the development and implementation of practice policies and procedures.
    • Participate in audit and quality improvement initiatives to enhance patient care.

  5. Collaboration and Communication:

    • Work closely with GPs, healthcare assistants, and other members of the practice team to coordinate patient care.
    • Liaise with external agencies and services, such as hospitals and community services, to ensure continuity of care.
    • Participate in team meetings, case conferences, and professional development sessions.

  6. Professional Development:

    • Stay current with nursing practice developments, guidelines, and regulations.
    • Participate in ongoing training and continuing professional development (CPD) activities.
    • Supervise and mentor junior staff and student nurses as required.
