The right candidate should be forward thinking, able to work on their own as well as part of the team as the role will involve being responsible for risk assessment and management, care planning and care delivery within a team approach.

The Haringey CRHTT is an innovative and creative multi-disciplinary team, always looking to improve the service it provides to its community. Accredited with the highest award by the Home Treatment Accreditation Scheme, operated by the Royal College of Psychiatrists for excellent service to our patients, the team is looking for a talented Band 4 Associate Mental Health Worker who will be responsible for assessing a mental health crisis and delivering safe and effective home treatment for people in accordance with their care plan and risk assessment.

The right candidate should be forward thinking, able to work on their own as well as part of the team as the role will involve being responsible for risk assessment and management, care planning and care delivery within a team approach. The post holder should be confident in assessing and managing mental health and social needs and be able to deliver a range of therapeutic techniques. Devising care plans in partnership with the people that are referred, and their carers/families is essential. The post holder will work collaboratively with clients, carers and multiple other agencies to provide effective care to optimize and maintain clients' mental health and overall functioning, ensuring needs are met.

The post holder will work under supervision of senior nurse practitioners and directly with people with severe mental health needs and their families, carrying out assessments (including risk management) and effective treatment and/or interventions, with the aim of helping people participate in normal family and community activities and to facilitate positive outcomes of care in relation to people's mental health needs.

The post holder will participate in the delivery of the agreed aims and objectives of the service and identify factors which may inhibit these from being achieved.

The post holder will commonly provide interventions and treatment for people presenting with co-morbid mental and physical health care needs, as well as complex biopsychosocial presentations. The team is passionate about promoting good mental health and wellbeing. The post holder will provide supportive care management to an allocated group of service users as delegated by the Team /Deputy Manager or registered practitioner.

The post holder will work with the community team in line with service needs.
