Nottingham CityCare Partnership CIC is seeking a motivated Assistant Practitioner to join their Front Door Integrated Discharge Team, located at Queens Medical Centre. This role involves working within the Emergency Department and Assessment Areas to support the Comprehensive Geriatric Assessment (CGA) process, collaborating with a multidisciplinary team including nurses, physiotherapists, and occupational therapists. The primary responsibilities include conducting comprehensive assessments for patients, developing discharge plans, promoting patient independence, and effectively communicating with clients, families, and other healthcare professionals. The position requires strong organisational and problem-solving skills, with the ability to manage a caseload of complex and frail older patients.
