The EWS Care Navigator role at Provide CIC in Colchester involves assessing individuals' readiness for lifestyle changes via telephone consultations. The primary responsibilities include identifying the support needed to improve self-management, organizing access to local services, and signposting clients to specialist services for emotional support and independent living. Additionally, the Care Navigator will follow up with clients post-intervention to discuss further steps and provide evidence-based information on health issues related to lifestyle choices, such as smoking, physical inactivity, and unhealthy eating.

The role also entails undertaking any delegated tasks as required and performing other functions aligned with the competencies of the position. Previous applicants for this role are advised not to reapply.
